AccentHealth HONORS Program

OFFICIAL SWEEPSTAKES RULES AND REGULATIONS - 2011

  1. ELIGIBILITY. The AccentHealth HONORS program sweepstakes is open only to a practice showing AccentHealth programming. Entrants must be legal residents of the United States, living in the United States who are at least 18 years of age, and are employed by a practice showing AccentHealth programming. Employees of AccentHealth LLC, sales representatives, distributors, advertising/promotion agencies and the immediate families or household members of each are not eligible.
    By entering, participants agree to release, discharge and hold harmless AccentHealth LLC, (hereafter "we" or "AccentHealth") and our representatives from all damages which may arise out of participation in this program, or out of the acceptance, use, misuse or possession of the prize. By participating, participants agree to these Official Sweepstakes Rules and to the decisions of the judges, which are final and binding in all respects. Void where prohibited by law, rule or regulation. All federal, state and local laws and regulations apply.

  2. THE PRIZES. Monthly prizes for $100 gift certificates for various online or local services. The Grand Prize is a Patient Services Grant of $10,000 as further described in the HONORS Program Guide. Total approximate retail value of all prizes is up to $11,000.

  3. HOW TO ENTER. The HONORS program runs from March 1 to December 30, 2011. To enter, you may complete an Official Entry Form provided with the Monthly Program Guide and fax the form back to the fax number specified or mail to the address below. You may also download the form from www.accenthealth.com/honors. Entries must be received by the 30th of the month to be eligible for that month's drawing. Mail your entry in an envelope to: AccentHealth HONORS Program, 5440 Beaumont Center Blvd., Suite 400, Tampa, FL 33634. All contact information collected will be used solely for Sweepstakes purposes, winner notification and awarding of prizes. Properly completed online entries received prior to June 1, 2011 with a completed entry form will be entered into the Grand Prize contest.

  4. WINNER SELECTION. Odds of winning depend on the number of eligible entries received. AccentHealth will conduct a random drawing on or about the last day of each month to award the monthly prize. Monthly winners will be notified by phone within one week of winner selection. The Grand Prize winner will be selected from all eligible entries received at www.accenthealth.com/honors between March 1, 2011 and June 1, 2011. Winner will be selected by an AccentHealth panel of judges based on rationale for why Patient Services Grant is needed. The Grand Prize winner will be notified by phone on or about July 1, 2010. All prize acceptance documents must be signed and returned to AccentHealth within one week of receipt. If prize notification or prize is returned as undeliverable, prize may be forfeited and awarded to an alternate winner. Participants agree to abide by the terms of these Sweepstake Rules and the decisions of AccentHealth and the judges, which are final and binding on all matters pertaining to the Sweepstakes. Grand Prize winner agrees to launch grant activity prior to October 1, 2011.

  5. GENERAL. Subject to all federal, state and local laws and regulations. Void in Guam, the Virgin Islands, Puerto Rico and wherever else prohibited. Any taxes on the prize are the sole responsibility of the winners. No substitutions or transfers of the prize will be accommodated or permitted, other than those at the sole discretion of AccentHealth. By entering, each participant agrees to indemnify, release, defend, and hold harmless AccentHealth LLC and our representatives from all damages such participant may have arising out of participation in this program, or out of the acceptance, use, misuse or possession of the prize(s). By entering, each participant further agrees that, in the event there is any conflict or other inconsistency between the Official Rules and any promotional or marketing materials, e-mails, or announcements relevant to this Sweepstakes, these Official Rules will govern. AccentHealth reserves the right to modify these rules at any time for purposes of clarification. All entries become property of AccentHealth and will not be returned. By entering, participants agree to allow AccentHealth to use their name, office space redecoration, and likeness in promotional materials, AccentHealth programming and the programming of AccentHealth's broadcast partners without further compensation.

  6. DISQUALIFICATION. AccentHealth will not be held liable for (1) late, lost, damaged, incomplete, or illegible entries; (2) telephone, electronic, computer program, network, Internet, or computer malfunctions, delays or failures of any kind; (3) failed, incomplete, garbled, unintelligible, or delayed facsimile messages, whether human or technical in nature; or (4) any illnesses or injuries resulting from entering the giveaway. We reserve the right to disqualify any participant from the Sweepstakes. In the event that any portion of this Sweepstakes is compromised by causes beyond our control that, in our sole opinion, corrupts, or impairs the administration, security, fairness or proper play of the Sweepstakes, we reserve the right at our sole discretion to suspend or terminate this Sweepstakes or any part of the Sweepstakes.

  7. MONTHLY ANSWERS/WINNERS LIST: To obtain The AccentHealth HONORS Program answers, or a list of winners, send a self-addressed, stamped envelope to: The AccentHealth HONORS Program c/o AccentHealth, 5440 Beaumont Center Blvd., Suite 400, Tampa, FL 33634. Please specify "Answer" or "Winners List." Requests for the AccentHealth HONORS Program answers must be received by the 7th of each month for the preceding month's Entry Form's Question. Requests for the Monthly winners must be received by the 14th of each month for the preceding month's winners. Requests for the Grand Prize winner must be received by November 1, 2011.
